In the name of Allah, the Most Gracious, the Most Merciful
Answer:
If a person forgets certain words during the Adhan/Iqamah, then if he remembers during the Adhan/Iqamah or immediately thereafter, he should restart the Adhan/Iqamah from the words that he left out and finish as usual. There is no need to restart the Adhan/Iqamah from the beginning. For example, he forgot to say Hayya alas Salah during the Iqamah, however, he either remembers during the Iqamah or immediately thereafter that he left these words out. In such a situation, he will restart the Iqamah from Hayya alas Salah and finish it like normal. Nevertheless, if he remembers after a long period of time that he left out some words of the Adhan/Iqamah, then he should restart it from the beginning.
